Incident Reporting
ShiftFlex provides a built-in incident reporting system so employees can flag workplace issues during or after a shift, and managers can review and resolve them promptly.
Reporting an Incident (Employee)
You can report an incident tied to any shift you've worked:
- Open the shift detail for the relevant shift (from My Shifts).
- Tap Report Incident at the bottom of the screen.
- Fill in the incident form:
- Type — select the category that best fits
- Severity — select how serious the incident is
- Description — write a clear, factual account of what happened
- Tap Submit. Your manager receives a notification immediately.
Incident Types
| Type | Examples |

| Safety | Slip and fall, hazardous condition, near-miss, injury |
| Policy Violation | Harassment, misconduct, breach of company rules |
| Equipment | Broken machinery, damaged tools, vehicle issues |
| Customer | Aggressive customer behavior, complaint, theft |
| Other | Any incident not covered by the above |
Severity Levels
| Level | Meaning |
|---|---|
| Low | Minor issue, no immediate risk, informational report |
| Medium | Moderate concern, should be addressed soon |
| High | Serious issue requiring prompt manager attention |
| Critical | Immediate danger or emergency — escalate immediately |
Managing Incidents (Manager)
Go to Settings → Incidents to see all reported incidents for your organization. The dashboard badge also shows a live count of open incidents.
Reviewing an Incident
- Select an incident from the list to view its full details: who reported it, which shift it's linked to, type, severity, and description.
- You can update the status as you work to resolve it.
- Add resolution notes to document your actions.
- Tap Resolve when the incident has been addressed. The reporter is notified.

Critical incidents
For Critical severity incidents, managers are notified immediately via push notification. If the situation involves immediate physical danger, call emergency services first before using the app.
